Checklist for Home-schooled Students

Checklist for Admission Documents for Students in Accredited Programs

  1. A completed Georgia Perimeter College application. (Be sure to designate the appropriate GPC campus for your high school.)
  2. A $20 application fee.
  3. An official high school transcript submitted in a sealed envelope from the high school or electronically submitted by the high school.
  4. A current Certificate of Immunization (There are very specific requirements from the Board of Regents—don't wait until the last minute.)
  5. An official SAT or ACT score report sent directly to GPC. The college code for the SAT is 5711 and the college code for the ACT is 0806.
  6. A signed Parental Consent Form.
  7. All students must provide verification that they are legally in the United States. More information on this requirement can be found on the Admissions web pages, Verification of Lawful Presence .
  8. Official AP scores (if any) for credit and placement into advanced classes.

If born or educated outside the U.S., documents certifying U.S. citizenship or immigrant/non-immigrant status (resident alien card, naturalization certificate, U.S. passport) must be presented to the Office of International Student Admissions and Advising on the Clarkston or Dunwoody campuses or the Enrollment and Registration Services Office on the Newton, Alpharetta, or Decatur campuses. Checklist for Admission Documents for Home-schooled Students

  1. A completed Georgia Perimeter College application.
  2. A $20 application fee.
  3. A Home School Portfolio Summary form along with supporting documentation. The Summary form and documentation must substantiate completion of all College Preparatory Curriculum units equivalent to a Georgia public high school sophomore. Failure to submit adequate supporting documentation will delay processing. Official transcripts from any regionally accredited public or private high schools attended should be submitted with the portfolio.
  4. A copy of the ‘Declaration of Intent to Utilize a Home Study Program’ that was submitted to the local school district. Information on this form may be verified with the local school district.
