ELIGIBILITY FOR COVERAGE

All International students or scholars with a current passport and a non-immigrant visa temporarily residing outside their home country while actively engaged in education or educational research at the University of South Alabama are required to be insured under this Plan.

Exemptions from participation may be granted at the direction of the administrator for students who are already covered under certain government or embassy sponsored plans. To apply for an exemption, the student should contact the University of South Alabama Office of Enrollment Services.

To be an Insured Person under the Policy the student must have paid the required premium. The Company maintains its right to investigate student status and attendance records to verify that the Policy eligibility requirements have been met. If and whenever the Company discovers that the Policy eligibility requirements have not been met, its only obligation is a pro-rata refund of premium.

DEPENDENT COVERAGE
Eligible insured students may also purchase dependent coverage. Dependents must be enrolled on the date the student is enrolled or within 31 days of birth, marriage, or arrival in the U.S. An Eligible Dependent may be the Eligible Participant’s lawful spouse and/or his/her unmarried children under age 19 who are chiefly dependent upon the Eligible Participant for support and maintenance. The term “child/children” includes a natural child, a legally adopted child, a stepchild, and a child who is dependent on the Eligible Participant during any waiting period prior to finalization of the child’s adoption. The Eligible Dependent is one who has a similar visa or passport accompanies the Eligible Participant while that person is engaged in international educational activities; and is temporarily located outside the Eligible Participant’s Home Country as a non resident alien. Dependents of an Eligible Participant mush possess a valid passport and a proper visa (F-2, J-2, M-2). A “newborn” will automatically be covered for Injury or Sickness from birth until 31 days old, provided that the student is covered under the plan. Coverage may be continued for that child when the Company is notified in writing within 31 days from the date of birth and additional premium has been paid.
Dependents must be enrolled for the same term of coverage for which the Insured Student enrolls. Dependent coverage expires concurrently with that of the Insured Student. Applications for dependent coverage are available at the University of South Alabama Office of Enrollment Services.
