INTERNATIONAL STUDENT HEALTH PLAN

Promotion of good health for students has always been our concern. This brochure summarizes how the Student Health Insurance Plan works, what it covers and how the plan will help you with medical costs. After you’ve read about the Student Health Insurance Plan, Keep these important facts in mind:

  • Keep your insurance card with you at all times, and show it to the doctor or hospital when you seek medical treatment.

  • Learn about the University’s Student Health Services (SHS), its location, hours of operation, and the types of services it offers. If possible, go first to your university’s SHS for treatment during their regular business hours of operation. SHS can help you locate off-campus medical providers when you need additional care or specialists.

  • You may choose any doctor you wish, but if you use a Hygeia/First Health PPO provider, it may save you money. You can locate them on the web by logging in at www.hthstudents.com or by calling (888) 350-2002. Please see the section titled “ Hygeia/First Health PPO Network” in this document for more information.

  • Complete all forms you are sent accurately and respond promptly to requests for information from the claims administrator.
PREMIUMS
Premiums are collected annually at the beginning of the Fall Semester. The starting date for the Policy for the 2005-2006 school year is August 1, 2005. Students who begin their studies in the Spring or Summer Semesters will pay a prorated premium which will provide coverage for the remainder of the policy year. Premiums are deemed received when paid to the University of South Alabama or when received in the Office of the Administrator. Unless extended, the ending date of the Policy will be July 31, 2006.
