International Education Forms

All education/study abroad programs require specialized application forms for academic advising, liability release, insurance coverage, etc. Please contact your program of interest as soon as possible to secure the proper forms.

Contact the USA Office of International Education for the appropriate forms used by USA academic units and OIE in approving all study abroad options. ALL study, internship programs, etc. must be pre-approved by the USA Office of International Education.

included in IS fee for just study abroad activities must have other US based comprehensive major medical insurance.
