**ELECTRONIC DIRECT DEPOSIT FORM DEADLINES**
SUMMER SEMESTER 2008 - MAY 16, 2008
FALL SEMESTER 2008 - AUGUST 15, 2008
SPRING SEMESTER 2009 - JANUARY 9, 2009
For timely receipt of your financial aid you will need to sign up for Electronic Direct Deposit (EDD). EDD permits delivery of your aid (after deduction of University charges) directly to your checking account. For those who do not participate in the Electronic Direct Deposit Program, your checks will be MAILED to your permanent address on file. We will mail checks within five business days of drop/add date.
To Receive Your Aid Through Electronic Deposit:
1. Have a checking account and preprinted checks in your name with a financial institution that participates in electronic deposit transactions - this includes most banks, savings and loan institutions, or credit unions. Check with your financial institution for details.
2. Submit or mail a completed EDD Authorization Form and a voided check to the Office of Student Accounting.
3. Complete all Federal Financial Aid eligibility requirements as specified by the Financial Aid Office, including pre-loan counseling and loan promissory notes.
4. Ensure that you are enrolled in the appropriate number of credit hours and that you have cleared any University or Financial Aid holds which would prevent disbursement, before the Official University Drop/Add date.
5. Ensure that the University always has your current permanent address. To make any address changes please log on to PAWS or write the Registrar's Office at the following address:

6. Complete only one EDD Authorization Form at the beginning of your enrollment at USA, unless you change your bank or checking account status during the period you are receiving Federal Financial Aid.
