Reflection Logs

Please fill out the front of the reflection log and have the supervisor for where you did your service work sign it.

Please fill out the questions on the back of the reflection log and use extra paper when necessary to answer all of the questions. Please remember that you must answer all of the questions or your log will be considered incomplete and you will not receive credit for your hours.
