Reporting an Incident

Any person wishing to initiate a complaint regarding any Northeastern student may do so by submitting an online report to the Office of Student Conduct and Conflict Resolution.  All complaints or incidents referred to the Office of Student Conduct and Conflict Resolution will be reviewed to determine if it is possible that a violation of the Code of Conduct could have occurred. If so, the case is assigned to a staff member for review.  The office does not investigate incidents. Therefore, some incidents submitted may be referred back if additional information is necessary.  Once a case is assigned and necessary documentation is received by the office, the student will be notified of the incident, charges, and a meeting time to discuss the resolution of the case.

Nature of this report: select - general incident report

Date of incident: use the calendar tool and select the date the alleged incident occurred

Time of incident: if applicable, please choose the time of the incident - for academic integrity cases or ongoing violations, please leave this blank

Location of incident: Please choose the location the incident occurred (building, in a classroom, etc).  For a violation that occurs "online" (i.e. social media/e-mail) please choose "online".  For any academic integrity violation (online or on ground) please choose "class".  For violations that occur off campus, please choose the area that best describes where the violation occurred (off campus, mission hill, fenway, etc).

Specific location of incident: Please give the specific location of the incident (e.g. Room 212, 1313 Mockingbird Lane - Apartment 4, etc). For any academic integrity violation, please list specific location as the course (e.g. CS 1234, MATH 1234, etc).
