The State of Alabama requires that a Vendor Disclosure Statement be completed for all proposals, bids, contracts, or grant proposals in excess of $5,000. Once completed by the vendor or subcontracted, the Disclosure Statement must be filed with various State Agencies. The Purchasing Department will coordinate the distribution of these completed statements to the appropriate state agencies.
