NIH Announces Changes in Business Process on PHS 416-1 Grant Applications, PHS 416-9 Progress Reports and Prior Approval Requests for Individual Fellowships.
The National Institutes of Health announced on October 14, 2008 changes regarding the signatures of the Applicant (Individual Fellow) and Sponsor(s) as part of applications, certain post-submission information, progress reports, and post-award prior approval requests and the corresponding implementation of a new Institutional compliance requirement to secure and retain the Individual Fellow and Sponsor(s) signatures as part of the institutional review/approval process.Please click on the link below and download the USA Certification and Assurance form for the Individual Fellowship applications. In addition, the University of South Alabama will also ask the applicant to read the Ruth L. Kirschstein National Research Service Award Payback Agreement. Please also download this agreement, read it and attach it to the back of the USA Certification and Assurance that is required by the National Institutes of Health. The University will require both of these documents be included in your PHS 416-1 Grant Applications. Please contact the Office of Health Systems Grants Administration and Development with questions.
