All forms must be turned in to Sports Clubs Coordinator.
Required from every member of every club at beginning of season/academic year:

1 - Sports Club Information Form
2 - Liability Waiver Form
3 - Pre-participation Physical Examination Form

Required from every member of clubs involving physical contact:

1 - Medical Exam Form

Required from President or Advisor prior to each off campus event:

1 - Trip Form

Required from each club Treasurer at the the beginning of each season/academic year.

1 - Sports Club budget request

Internal club forms for use by club officers.

1 - Checklist of responsibilities
2 - Equipment check-out form

Required from each club at the beginning of each season/academic year AND as new members are added.

1 - Sports Club roster

Required from each club at the end of each semester in which the club is active.

1 - Semester Report Form
