Student & Scholar Health Insurance at USA

Student & Scholar Health Insurance for the 2014/2015 school year is provided by UnitedHealthCare Student Resources (UHCSR).  Please read all of the information below before deciding whether to enroll.  

Eligibility Requirements & Who is Eligible?

All full-time USA undergraduate students are eligible to purchase the student health insurance plan.  Part-time, USA undergraduates enrolled in at least 6 credit hours per semester on campus (in a traditional classroom setting) and USA graduate students enrolled in at least 3 credit hours per semester on campus (in a traditional classroom setting) are eligible to purchase the student health insurance at the part-time rate; students taking additional credit hours for a total of 12 credit hours are eligible to purchase the plan at the full-time rate.

* Students must actively attend classes for at least the first 31 days after the date for which coverage is purchased.  Home study, correspondence, Internet and television (TV) courses do not fulfill the Eligibility requirements that the student actively attend classes.  UHCSR maintains its right to investigate Eligibility or student status and attendance records to verify that the policy Eligibility requirements have been met.  If UHCSR discovers the eligibility requirements have not been met, its only obligation is to refund the premium and cancel coverage.

Undergraduate Students (Domestic) and Non GA Graduate Students

Who is Eligible?

*Students majoring in the above programs are eligible and must purchase the university endorsed health plan, unless proof of comparable coverage is furnished to your program.

Please note: Health Insurance Coverage For Those Eligible Students Enrolled In A “Health Care Program” Must Be Purchased From The USA Student Health Center and Enrollment Information Sent on A School List.   STUDENTS CANNOT ENROLL ONLINE!

Please note:  If you are a Nursing student and all of your classes are Online, you will not be eligible to purchase the Student Health Insurance that is offered to the students attending classes on campus.  Please refer to the Online Graduate Student link.

Online Graduate Students

Please note:  Golden Rule is a part of United Health Care and they offer short term individual policies.  They are not affiliated with our university or our student health insurance plan.

International Degree Seeking, Exchange or ESL Students

All international students  MUST purchase the university endorsed Student Health Insurance plan from United HealthCare Student Resources (UHCSR).  (All international students are automatically  enrolled in this insurance plan upon registration.)

The University of South Alabama will provide a waiver (exemption) of this requirement to purchase the UHCSR plan under only 2 conditions:

1.  You are attending USA on a Scholarship/Sponsorship and are insured by your government-sponsored U.S. based health insurance plan from Saudi Arabia, Kuwait, UAE, Oman and Iraq.  ( Please Note:  A copy of your Financial Guarantee letter/ Sponsorship letter and copy of your health insurance information must be submitted to Rhonda Baxter (at the USA Student Health Center) before classes begin each semester.)  All necessary documents can be submitted via email to: rbaxter@southalabama.edu .

2.  You have American group health insurance coverage under a US employer either as an employee or as a dependent.  In this case, you must provide proof that medical evacuation and repatriation benefits are also included on your policy.

Waiver Applications must be received before classes begin each semester. If the necessary documents are not received before classes begin, the student will be responsible for all unpaid International Insurance fees billed to their student account.   This fee cannot be removed from your student account.

Post Doctoral Trainees at USA

All USA Post Doctoral Trainees eligible for single coverage health insurance thru the department in which he/she will be working, “Must” complete the 2014/2015 Enrollment application and return it to your department for submission to the USA Student Health Center, Student Insurance Office.  International Post Doctoral Trainees under the J EV Program are also subject to the US Dept of State Exchange Visitor regulations as outlined below.

*If you are unsure of whether you are eligible for insurance coverage please contact the department in which you will be working.

BMS Graduate Students

Dependent Insurance Coverage

Eligible students/scholars who do enroll may also insure their Dependents.  Eligible Dependents are the spouse and unmarried children 19 years of age or 23 years if a full-time student at an accredited institution of higher learning who are not self-supporting.  Dependent eligibility expires concurrently with that of the Insured student/scholar.
