Book or Other Material Reserve Request

Use this form to place books, book chapters, videos, CD's, DVD's and any other materials owned by the library on reserve in the library. You may also use this form to place book chapters on e-reserve. If you wish to place a journal article on reserve, use the Journal Article Reserve Request form. If you wish to place CD's, cassette tapes, or records on e-reserve, use the Electronic Audio Reserve Request form. If you wish to place material you own on reserve, you can print out the Reserves Printable Form (MS Word) or the E-Reserves Printable Form (MS Word).
