Proxy Patron Request

Faculty, staff, and administrators have the option of having a student check out library materials for them. The student uses his or her own ID card, but the materials are checked out in the name of the Mansfield employee who has authorized them. For a student to be authorized, the Mansfield employee must fill out the Proxy Patron Request Form and return it to the Circulation Desk at the library.
