Steps for Respirator Use

1. Request for Respirator Use
Department completes and submits Review for Respirator Use Form  for:

  • All new respirator users
  • Annual Tight Fitting respirator users
  • Users with changes to their job duties and/or physical attributes that could result with an improper fit.

2. Medical Review
Users submit the Respirator Medical Questionnaire directly to the OCCMED clinic.

3. Training
Training is required for initial use and annually thereafter for all respirator users.
Search for the appropriate course code in myTraining

4. Fit Test
Fit Tests must be completed for initial use and annually thereafter with the respirator model individual will be using.
Call EHS: 352-392-1591 to schedule appointment – ask for Jeff Daniel or Tom Ladun
