VA Certification Request Card - Instructions
Certification is not automatic and must be requested prior to the start of each semester via the VA Certification Request Form . Please adhere to the requested due dates below to ensure prompt payment of your benefits.
VA Certification Request Card Due Dates:
June: Fall SemesterOctober: Spring Semester
March: Summer A, B & C Sessions
Information Requested:
1. FAU Student Number – Social Security Number2. Name
3. Local Mailing Address - (This will be the address checks
will be mailed to) ***Address Changes must be reported to the VA Office to ensure receipt of benefits***
4. Daytime Phone Number
5. E-Mail Address ***Must be an FAU e-mail address***
6. Major Seeking - Benefits will not be authorized without this
information. If you change your major in the future, please notify the VA Office. If this is a new major, indicate by selecting YES otherwise select NO.
7. Deferment – Indicate “Yes” for requesting a deferment and
“No” for declining a deferment. You can only request one deferment per academic (fall, spring, or summer). Requesting a deferment entitles you to extend year payment of Tuition and Fees up to 60 days from the first day of class. This time allows you to receive VA Benefits or pay Tuition and Fees out of pocket. Chapter 31 is excluded.
8. Indicate how many months of VA Benefits you have left
if it is 9 months or less.
9. Indicate what chapter you are currently receiving benefits
under.
10. Indicate what degree you are seeking. ***You will only be
certified for courses required toward your degree of study***
11. VA File Number – Chapter 35 Only. Excluding this number
may delay processing the claim.
12. Enter Credit Hours, which Semester, and what Year.
13. If you are in a Special Program, indicate the one that
applies.
14. Sign and Date
