Change of Instruction

Eligibility:  

1. You have used your GI Bill Benefits before
2. Transfer Student
3. Major/Program Changes
Returning to FAU after NOT using benefits for over 1 year

If one of the above qualifications applies to your situation, complete the following forms with Florida Atlantic University:

1. Complete the VA Certification Request   Form.
2. Complete Form 22-1995 - Request for Change
of Program or Place of Training.
3. Complete a Prior Education and Training Questionnaire
4. Provide a copy of the DD-2384 - Notice of Basic Eligibility.
5. Provide a copy of Certificate of Eligibility from the VA.

Important: Incomplete paperwork will cause a delay in processing times as well as receiving your benefits.  Please submit forms to the following location:
