Application Instructions
Eligibility:
1. Enlisted or entered Active Duty for the first time on orafter September 11, 2001. You must possess an Honorable Discharge.
Prior to completing the necessary forms for Chapter 33 benefits, you may want to determine your eligibility by calling 1-888-GIBill-1 (1-888-442-4551). This number is also a great source of additional assistance in reference to benefit amounts and processing times. For additional inquiries, you may want to view the following website: http://www.gibill.va.gov/.
Next, you may complete the following forms with Florida Atlantic University:
1. Complete the VA Certification Request Form.2. Complete Form 22-1990 – Application For Education Benefits.
3. Complete a Prior Education and Training Questionnaire
4. Provide a copy of the DD214 - Member 4 (Honorable
Discharge).
Important: Incomplete paperwork will cause a delay in processing times as well as receiving your benefits. Please submit forms to the following location:
