Change of Instruction
Eligibility:
1. You have used your Chapter 33 benefits before2. Transfer Student
3. Major/Program Changes
4. Returning to FAU after NOT using benefits for over 1 year
If one of the above qualifications applies to your situation, complete the following forms with Florida Atlantic University:
1. Provide a copy of Certificate of Eligibility from the VA.2. Complete the VA Certification Request Form.
3. Complete Form 22-1995 - Request for Change of Program
or Place of Training.
4. Complete a Prior Education and Training Questionnaire
5. Provide a copy of Form DD-214 - Member 4
(Honorable Discharge)
