Application Instructions
Eligibility for Children:
Parent * a permanent & total service-connected disability, orMust Have: * died as the result of a service-connected disability, or
* died while totally & permanently service- connected
disabled, or
* died in the line of duty while a service member
Eligibility for Spouses:
The veteran must have permanent and total service-connected disability.
Eligibility for Surviving Spouses:
The death of the veteran must be service-connected or veteran was permanently and totally service-connected disabled at time of death.
Prior to completing the necessary forms for Chapter 35 benefits, you may want to determine your eligibility by calling 1-888-GIBill-1 (1-888-442-4551). This number is also a great source of additional assistance in reference to benefit amounts and processing times. For additional inquiries, you may want to view the following website: www.gibill.va.gov.
Next, you may complete the following forms with Florida Atlantic University:
1. Complete the VA Certification Request Form.2. Complete Form 22-5490 – Application For Survivors' and
Dependents' Educational Assistance.
3. Complete a Prior Education and Training Questionnaire
