These are some of the common forms used for administrative requests, or information sources.

FAAS FORMS (please scroll down...)

• Undergraduate Academic Petition Form (undecided majors only)  - Used to request exception to undergraduate requirements or rules (not for course equivalencies). Also used to request to drop a class after the deadline, reinstatement into FAU, and total withdrawal from a specific term. Click here for proper instructions of using this form.
  

• Late Withdrawal Course Instructor Documentation Form - Used to request a late withdrawal from a course.

• FAAS Change of Major Form - Used to request a change of major within the same college a student is currently enrolled within or a different college that a student wish to pursue.

• CLAST Exemption Form  -  Used when a student has attained exemption from either the math or English portions of the CLAST exam by alternative means.  Complete and have signed by your Freshman Academic Advising Services advisor, then submit to the Testing and Evaluation office.
  

• AA Degree Application - Used to apply for your AA degree (check FAU's academic calendar for due dates).

UNIVERSITY FORMS

• Application for Admission to FAU  - Used to apply to FAU's undergraduate degree programs.

• Enrollment Verification Form  - This is a Registrar form, allowing you to obtain documentation from the Registrar's office concerning your enrollment status at FAU.

• Forgiveness Policy Form  - FAU form that allows students to "forgive" prior grade on a repeated course. 

• Gordon Rule Evaluation Form  - Used by the Registrar's office, to apply transfer work toward the state's Gordon Rule requirements.

• Immunization Form  - This Student Health form is for the required proof of immunization for all new FAU students.

• Transient Student Form  (use Transfer Services link)  - Used for taking courses as a visiting student at other Florida universities and community colleges.   This link is to the electronic version, allowing you to submit the form by web, and eliminating the need to hand-carry the form.  The electronic form is usually processed in 1 - 2 working days.  A paper version is available within the Freshman Academic Advising Services or Registrar's offices, if needed.
