International Student Health Insurance Requirements

All international students are permitted to enroll at Florida Atlantic University (FAU) after demonstrating they hold medical insurance coverage which meets the guidelines set by the Florida Board of Governors Regulation 6.009 and FAU Regulation 4.009. Misrepresenting or willfully failing to maintain appropriate medical insurance coverage will result in serious consequences for the student.

F-1 and J-1 students can comply with this requirements in two ways:

1.    Purchase the policy available through FAU from the Insurance for Students

2.    Provide proof of acceptable alternate medical insurance plan and completion of the 
Alternate insurance Compliance Form.

The Office of Student Health Services monitors compliance for F-1 and J-1 students. Payment receipts for the FAU-sponsored plan (Insurance for Students) and Alternate Insurance Compliance Form should be submitted to:

Student Health Services will also be responsible for removing insurance holds (MM holds) for international students in F-1 and J-1 status. Students who are not in F-1 or J-1 status and have an MM hold on their account should contact ISSS to make the appropriate changes .    

ISSS monitors health insurance compliance for all other J-1 and J-2 visa holders who are not enrolled for courses:

1.    J-1 Students who completed their program of study and are currently engaged in   
Academic Training

2.   J-1 Scholars (Short-term Scholars, Research Scholars, Professors, and Specialists)

3.    J-1 Student Interns

4.   J-2 dependents

All J Exchange Visitors are required by FAU regulation (4.009) and the U.S. Department of State (22 CFR Part 62.14) to have appropriate health insurance in effect for the entire duration of their J program. J-1 exchange visitors who are eligible for FAU employee benefits can sign up for one of the employee group health plan options and must contact ISSS to purchase the coverage for medical evacuation and repatriation (not included in the group health plans). All other J Exchange Visitors must provide adequate proof of health insurance to ISSS and have two options:

OPTION A: CHOOSE ONE OF THE FOLLOWING FAU-APPROVED PLANS

1.    The Florida Atlantic University Student/Scholar plan.

2.    The Scholar ICS discount plan.

3.    The Scholar Care plan.

Proof of Enrollment: Submit a copy of the payment receipt to ISSS.

OPTION B: SHOW PROOF OF ACCEPTABLE ALTERNATE INSURANCE COMPLIANCE

  1. Exchange visitors who choose Option B must provide a completed Insurance Compliance Form for J-1 Scholars and J-2 dependents (see below).
  2. The policy must cover the full period of the period listed on the DS-2019. If the DS-2019 is issued for a period longer than one year, the policy must be purchased for least one year at a time and renewed thereafter.
  3. The insurance company must have a U.S. based claims agent address and contact phone number.
  4. ISSS will not review individual policies to determine eligibility.

Proof of enrollment: Provide the Insurance Compliance Form (see reverse side or second page), completed and signed, to ISSS within 10 days of arrival in the U.S. Submit this information to:

2011-2012 Insurance Information

The FAU-approved insurance plan for the upcoming academic year is not yet available for review. If you have any questions regarding health insurance please contact Student Health Services which is located in the office SS8, 222L (above food court in breezeway).

Immunization Requirements

New FAU students are also required to provide proof of immunizations prior to enrolling in courses at FAU. Contact the Immunizations Office for further details.
