NOTE

The forms below are attachments to be submitted with the Undergraduate GER DAAF available from the Registrar's website.  Once you have the appropriate signature/stamp on the GER DAAF, submit it with the completed, applicable DAAF attachment below to the School of Education (3 pages total).

*BA/MA Students Only - Degree Audit Application Forms for BA/MA students include the undergraduate GER DAAF, the BA attachment and the MA DAAF. The MA DAAF must be signed by the program coordinator of the major (e.g. Biology, Mathematics) and be attached to the complete DAAF. Turn in the complete DAAF to Room 1000W.
