All referees are required to fill out the Human Resources Independent Contractor Determination Form. Please only fill out the highlighted areas of the form. Any referee that has filled out the form prior to Jan. 1, 2010, must submit a new form for the 2010-11 season. Payment for any services cannot be finalized until the form has been submitted.
