Here are some forms and checklists to help you prepare for your candidacy exam.

• Candidacy Check List - This list should help you keep track of your paperwork and responsibilities surrounding the Candidacy exam.
• Candidacy Notification - Please provide the Math office with a copy of the notification form for your Candidacy Exam at least one week before your exam date, as we must file this with the Graduate Office in the College of Arts and Sciences.
  
• Candiday Form - Fill out before you take the candidacy exam.

***Don't forget! The foreign language requirement must be satisfied before taking the Ph.D. candidacy, and after choosing a Dissertation advisor.
