IRS Form 1098-T, Tuition Statements for 2011, accessible on BANNER self-service

Follow The Instructions Below:
1. Click on the BANNER Link on the left
2. Select the Self Service for Students
3. Click on BANNER Self Service
4. Enter your User ID(V#) and PIN, then select the Login button
5. Select the Student Tab at the top of the page
6. Select Student Account
7. Click on the Tax Notification Tab
8. Enter Tax Year: 2011 and click on the Submit button

Your 2011 Facsimile of form 1098-T will be displayed. You can print this form to use in filing your 2011 Income Tax Return(s). An original completed form 1098-T Tuition Statement is being mailed to the student's permanent address of record in BANNER.

NOTE:

Please refer your tax related questioins to a qualified Tax Preparer.
