All fields are required unless otherwise specified. If you have multiple projects, please submit a separate request for each project.

  • Please enter your contact information. Use your campus email address, and enter either your 4 digit extension or your complete phone number with area code.




  • Include detailed location, and as much information as possible. Do not include a list of dissimilar projects; submit a separate request for each project.
  • How many people will be affected by this alteration/renovation? Enter '0' if none are affected.



  • For example, not while classes are in session or during the summer only.
  • Do you have access to funds that could be used to complete this project? , and I can contribute $
  • If self funded, must the encumbrances be spent in the current Fiscal Year?


  • Is this a space planning request?

All requests must be approved by the department head or chair, and by the dean, director or Provost of the department requesting this alteration or renovation.
