Students living in the United States:
Current KSU immunization policy requires that students attending the main campus must provide proof of the following immunizations:
1. Documentation of two MMR vaccines given after one year of age and separated by at least one month
2. Documentation of immunity as demonstrated by titers.
Methods of Documentation ( All documentation must contain name and date of birth):
1 .Download the immunization form for your healthcare provider to complete OR
2. Request a copy of the record from your high school OR
3. Copy the page from your Immunization book containing MMR dates

Mail, fax, or bring the records to University Health Services. DO NOT enclose the records with any other correspondence to the university. Be sure to include student's name and date of birth on the record.
All students born before January 1957 are exempt from the MMR requirement.
International students regardless of age, are required to submit the mandatory immunization form with part II completed.

Students from the following high risk countries are required to have a tuberculosis screening test (TB test – Mantoux type):

High risk countries have > 20 cases per 100,000 population.
This test must be completed and results submitted to UHS within 12 months prior to starting classes. Please download and submit a completed immunization form OR submit a copy of the TB screening information from your healthcare provider.
